I'm not familiar enough with HLH to comment on that but I can at least say that it would seem to complicate an already difficult diagnosis. AITL is one of the harder peripheral T cell lymphomas to treat. I think it might normally have as induction therapy CHOP, but that's less because the CHOP regimen is great against it and more because there isn't much else. I think the answer to your question-- removing the HLH for the moment -- is yes, probably, but I don't imagine the treatment success rate is all that high. Most PTCLs remain poorly understood. I think bone marrow transplants aren't uncommon.
How old is your brother? HLH is a childhood disease mostly, I believe?
